Honoring Our Children Project

The goal of the project is to reduce American Indian infant morbidity and mortality rates, while eliminating perinatal health disparities in Wisconsin. The model of care is through outreach, education, case management, maternal depression screening and referral, interconceptional care and consortia building.

Activities include: Outreach and education, home visits, transporation, phone usage, advocacy, support, Wadiswan – Nest incentive program, program advisory committee and local consortia, and tribal and non-tribal agency referrals.
